KVKK Application Form
Pursuant to the Communiqué on the Procedures and Principles of Application to the Data Controller

To exercise your rights listed in Article 11 of Law No. 6698 on the Protection of Personal Data, you can send us a written application containing the information below. Details of your rights are set out in the KVKK Privacy Notice.
What your application must include
- Name, surname and, for written applications, your signature,
- Turkish ID number for citizens of the Republic of Türkiye; for foreign nationals, nationality and passport number or ID number if available,
- Residential or business address for notifications,
- Email address, phone and fax number for notifications, if any,
- Subject of the request: which right you wish to exercise, with an explanation,
- Any information and documents supporting your request.

For applications by email, you must use an email address that you have previously given us and that is in our records, or send your application with a secure electronic signature or mobile signature. Please write "Information Request under the Personal Data Protection Law" in the subject line of the envelope or email.
How your application is concluded
Your application will be concluded free of charge as soon as possible depending on the nature of the request, and within 30 days at the latest. If the process requires an additional cost, the fee schedule set by the Personal Data Protection Board may apply. We may ask you for additional information to verify your identity.
